The application form for employment consists of 4 pages.
- First, write today’s date, then write your full name.
- Write the position for which you are applying and indicate the YMCA branch location.
- Circle your availability. Write your home address, email address, and phone number.
- Next, you should check some boxes next to a set of questions.
- On the second page, provide detailed information about your employment history, education, certifications, training, and additional skills.
- Indicate if you have performed military service.
- Then, provide three references.
- Answer a few questions in the Criminal Record section.
- Read the Statement From Applicant on the fourth page.
- Print your name, sign below, and write today’s date.
You can now submit this form to YMCA.
