The Party City application form for employment consists of 2 pages.
The first page has Personal Information, Employment Desired, Customer Service, Work Schedule, Education or Equivalent Experience, General and Work Experience sections.
Write your full name, email address, home address and phone number.
Then state the position you are applying for and the salary you desire.
Then answer some questions about your employment status.

Then indicate the days and hours you would be able to work.

At the bottom of the first page, you should list 3 of your working experiences.
On the second page, you should list 3 of your work experience references, provide their telephone numbers, and your supervisor’s position and company.
Then answer the remaining questions on the second page.
Lastly, read the Authorization section. Write the date and sign the paper if you agree.
Then the form will be completed.
