The Party City application form for employment consists of 2 pages.
- The first page has Personal Information, Employment Desired, Customer Service, Work Schedule, Education or Equivalent Experience, General and Work Experience sections.

- Then state the position you are applying for and the salary you desire.
- Then answer some questions about your employment status.

- On the second page, you should list 3 of your work experience references, provide their telephone numbers, and your supervisor’s position and company.
- Then answer the remaining questions on the second page.
- Lastly, read the Authorization section. Write the date and sign the paper if you agree.
